Citizenship ID Process
If you are a member of the Kelly Lake Cree, As'in'î'wa'chî Ni'yaw Nation, and you’d like an official identification card, please review the following information and follow the steps outlined below.
Before KLCN can issue a citizenship ID card, your identity as a citizen of the Nation must be verified. If you have any questions, would like to complete your request by phone, or if you require other assistance, please contact our Citizenship Administrator at citizenship@kellylakecreenation.com
Check the Registry
KLCN keeps track of genealogy and citizenship in a specialized database. Each member is assigned a unique registration number.
To find out whether your name is already included in the database, please email the Citizenship Administrator at citizenship@kellylakecreenation.com.
Request ID Card
If you already have a registration number, you can request your card directly via citizenship@kellylakecreenation.com.
Please submit the following information with your request:
Your full legal name
Alias or nicknames
Date of Birth
Photo (head shot with white or neutral background)
Copy or photo of your signature on a plain white surface
Your mailing address
Apply for Citizenship
If you are not already registered with KLCN, you will need to complete a citizenship application form. A valid long form birth certificate or Canadian Passport must be submitted for each individual.
